The patient was operated immediately. The lower end of the patellar button was removed and the quadriceps tendon was exposed. An anterior cruciate ligament-retaining multiple bur was used to drill the bone adjacent to the insertion point of the patellar tendon. Another bur was used to drill a small hole for K-wire introduction in the intercondylar eminence to reduce the fracture. The insertion points of the patellar ligaments were also drilled with multiple bur. Then the patella was pulled out and the fracture was reduced, and a tension band fixation was performed using an anatomical stainless steel plate. After thorough surgical debridement and direct repair of the anterior cruciate ligament, the displaced fracture was reduced and rigidly fixed using an anatomical steel plate (Double Medical Technology, China). Postoperatively, radiographs (Fig. 2 ) and computed tomography (Fig. 3 ) confirmed the anatomical reduction. The anterior/posterior drawer tests were negative. In a period of 14weeks until the final follow-up, the patient was able to squat and jump again. The Lysholm score achieved 92; his subjective evaluations were very good. However, the patient suffered a distal tibial fracture that required subsequent treatment. Eventually, he was evaluated for total knee arthroplasty. MRI (Fig. 4 ) and bone scintigraphy (Fig. 5 ) showed osteoarthritic changes of the knee joint and multiple bone removals, which were associated with previous distal tibial fracture. MRI (Fig. 4 ) showed the fracture lines of the osteoarthritic changes of the knee joint, which was an indication that the fractures were acomplicated osteoarthritic changes. Therefore, the patient was initially treated by partial weightbearing brace for 6weeks and complete weightbearing for 12weeks. He was then followed up for 6months and then annually. After that, he followed an outpatient rehabilitation program. The patient achieved a full recovery and returned to work.
